The Hospital Medicine Program Coordinator position is responsible for coordinating the day to day functionality and logistics within the Hospital Medicine Program. It supports program implementation, delivery and evaluation of services and assists in the operations of CCAD Hospital physicians by providing care coordination and discharge administrative facilitation.
Collaborates with hospital care team and hospital staff to facilitate the timely implementation of patient care plans.
Verifies primary care provider and notifies upon admission and discharge
Introduces the Hospitalist program to the patient and family and serves as the patient advocate. The goal is to “Meet and Greet” each new admission to the Hospitalist Service within 24 hours of admission
Obtains information relevant to the patient’s hospital stay from their primary care provider.
Determines potential barriers to discharge.
Ensure appropriate arrangements are in place to manage sickness absence
Collaborates with hospital discharge planning to ensure correct patient status, appropriate and safe discharge plan and disposition.
Communicates pertinent information regarding patient’s hospital stay with patient’s primary care, specialist as needed.
Assists with scheduling post-discharge follow up appointment with primary care provider or needed specialist.
Resolves post-discharge homecoming call issues.
Create and distribute the hospitalist schedule.
Assist in resolving complaints / concerns that are relayed from the Patient and Visitor Centre in line with CCAD Policy.
Serves as the liaison between discharged patients, families, and the hospitalist team. Handle the incoming calls, mail, and faxes including home health agency orders and pharmacy calls. Filters and distributes for the care providers so that priority items get a proper response.
Creates, edits, and distributes the hospitalist rounding and call schedules to appropriate parties.
Keep an updated contact list of the hospitalists, and forward appropriate correspondence.
Participates in data collection for quality improvement initiatives.
Reconciles entire Hospitalist service census with admission log and the previous day’s census; sends reconciled census to all units.
Ensure effective arrangements are in place to communicate both operational and business/strategic issues to all staff.
Compiles statistics for physicians, administration, and finance as needed. May gather data for other projects and performance improvement studies.
Consistently role models the highest level of customer service to all CCAD customers.
Coordinates with physician and prepares letters and forms as needed (IPC, Medical Reports, , Homecare, Work/School excuses, etc…)
Notifies physician of queries that require attention (insurance queries, credentialing needs, delinquent charts, etc…)
Performs other related duties as assigned.
